Differential Patterns of Adherence to Opioid Therapy in Opioid Naïve and Opioid Existing Patients With Different Age Groups

Limited data are available on the adherence to opioid therapy and the influence of different patient groups on adherence. This study examined the patterns of adherence in opioid naïve and opioid existing patients with varying age and gender. This retrospective cohort study was conducted using the prescription databases in tertiary hospital settings in Malaysia from 2010 to 2016. Adult patients aged ≥18 years, receiving at least two opioid prescriptions, were included and stratified into the opioid naïve and existing patient groups. Adherence to opioid therapy was measured using the proportion of days covered (PDC), which was derived by dividing the total number of days covered with any opioids by the number of days in the follow-up period. Generalized linear modeling was used to assess factors associated with PDC. A total of 10,569 patients with 36,650 prescription episodes were included in the study. Of these, 91.7% (n = 9,696) were opioid naïve patients and 8.3% (n = 873) were opioid existing patients. The median PDC was 35.5% (interquartile range (IQR) 10.3–78.7%) and 26.8% (IQR 8.8–69.5%) for opioid naïve and opioid existing patients, respectively. A higher opioid daily dose (coefficient 0.010, confidence interval (CI) 0.009, 0.012 p < 0.0001) and increasing age (coefficient 0.002, CI 0.001, 0.003 p < 0.0001) were associated with higher levels of PDC, while lower PDC values were associated with male subjects (coefficient −0.0041, CI −0.072, −0.010 p = 0.009) and existing opioid patients (coefficient −0.134, CI −0.191, −0.077 p < 0.0001). The suboptimal adherence to opioid medications was commonly observed among patients with non-cancer pain, and the opioid existing patients were less adherent compared to opioid naïve patients. Increasing age and a higher daily opioid dose were factors associated with higher levels of adherence, while male and opioid existing patients were potential determinants for lower levels of adherence to opioid medications.

Marine Protected Areas are designed to benefit the marine ecosystem and human coastal populations, but are they actually achieving both?

There are currently nearly 17,000 Marine Protected Areas (MPAs) across the globe. Broadly defined, they are patches of ocean in which human activities, often including tourism and fishing, are managed to ensure sustainability. Though large in number they only cover 11 million square miles – about eight per cent of the world’s ocean, according to Protected Planet, an online interface for the World Database on Protected Areas.

Organisations such as the WWF are now calling for 30 per cent of the ocean to be protected by 2030, a target which it says will ensure the most complete benefits for marine ecosystems and people. It is the latter that have generally been left out of the picture. While the conservation effectiveness of MPAs has been demonstrated in several ecological studies, their impact on coastal communities is less clear. The research that has been carried out in this area largely focuses on an MPA’s economic impact for these people, something Dr Morena Mills, a senior lecturer in conservation science at Imperial College London, recently discovered.

Working as part of an international research team, she helped examine 118 previous studies – mainly covering tropical MPAs in Asia and Europe – and separated out the positive and negative impacts identified for local people. Favourable impacts came just out ahead at 51 per cent of the total.

The bulk of these positive effects related to community involvement, increased income and a measure known as CPUE, or ‘catch per unit effort’, which refers to the number of fish caught in surrounding areas. Though it might sound counter-intuitive that MPAs (which often limit or prohibit fishing) result in increased catch, there’s a simple reason why. ‘Fish are different to us because the more they grow, the more eggs they produce,’ says Mills. ‘So if fish in no-take zones are allowed to grow bigger and produce more eggs, they start to spill over to the adjacent areas.’

These findings tally with a recent report commissioned by the WWF that points to huge net benefits for humans if ocean protection is extended to cover 30 per cent of the world’s ocean waters. The report predicts benefits in the range of $490billion and 150,000 full-time jobs in MPA management, (in some cases numbers reached as high as $920billion and more than 180,000 jobs).

But while this is a positive sign, Mills notes that this field of research is still limited. ‘There are very few studies that look at impacts on cultural diversity or mental health or other areas that we know are really important,’ she says. In addition, there are also the negative impacts to deal with. In particular, conflict within communities increased in 79 per cent of MPA cases examined and, although it was only analysed in 13 studies, the cost of fishing increased in every one.

As a result of this mixed bag, Mills now believes there needs to be more experimental or quasi-experimental studies to ensure that MPAs help both marine animals and humans. ‘Before and after impact control experiments are so important because they show you the real impacts of management.’

When you turn your information over to a private third party, you have given up the constitutional protections of privacy. Law enforcement routinely gets warrants for third party databases and aggregates the findings. Whether it is cell phone data, dna or any other kind of normally private data, once you give it to a third party, the bar is very low for law enforcement to access it.

I had warned the owner of gedmatch a month ago that this was going to happen, but he was in denial. He rolled immediately when given the warrant.

So, if you do not want law enforcement to get your information, do not provide it to third parties.

Introducing Detox, a React Native End-to-end Testing and Automation Framework

Detox is an end-to-end testing and automation framework that runs on a device or a simulator, just like an actual end user.

Software development demands fast responses to user and/or market needs. This fast development cycle can result (sooner or later) in parts of a project being broken, especially when the project grows so large. Developers get overwhelmed with all the technical complexities of the project, and even the business people start to find it hard to keep track of all scenarios the product caters for.

In this scenario, there’s a need for software to keep on top of the project and allow us to deploy with confidence. But why end-to-end testing? Aren’t unit testing and integration testing enough? And why bother with the complexity that comes with end-to-end testing?

First of all, the complexity issue has been tackled by most of the end-to-end frameworks, to the extent that some tools (whether free, paid or limited) allow us to record the test as a user, then replay it and generate the necessary code. Of course, that doesn’t cover the full range of scenarios that you’d be able to address programmatically, but it’s still a very handy feature.

End-to-end Integration and Unit Testing

End-to-end testing versus integration testing versus unit testing: I always find the word “versus” drives people to take camps — as if it’s a war between good and evil. That drives us to take camps instead of learning from each other and understanding the why instead of the how. The examples are countless: Angular versus React, React versus Angular versus Vue, and even more, React versus Angular versus Vue versus Svelte. Each camp trash talks the other.

jQuery made me a better developer by taking advantage of the facade pattern $('') to tame the wild DOM beast and keep my mind on the task at hand. Angular made me a better developer by taking advantage of componentizing the reusable parts into directives that can be composed (v1). React made me a better developer by taking advantage of functional programming, immutability, identity reference comparison, and the level of composability that I don’t find in other frameworks. Vue made me a better developer by taking advantage of reactive programming and the push model. I could go on and on, but I’m just trying to demonstrate the point that we need to concentrate more on the why: why this tool was created in the first place, what problems it solves, and whether there are other ways of solving the same problems.

As You Go Up, You Gain More Confidence

As you go more on the spectrum of simulating the user journey, you have to do more work to simulate the user interaction with the product. But on the other hand, you get the most confidence because you’re testing the real product that the user interacts with. So, you catch all the issues—whether it’s a styling issue that could cause a whole section or a whole interaction process to be invisible or non interactive, a content issue, a UI issue, an API issue, a server issue, or a database issue. You get all of this covered, which gives you the most confidence.

Why Detox?

We discussed the benefit of end-to-end testing to begin with and its value in providing the most confidence when deploying new features or fixing issues. But why Detox in particular? At the time of writing, it’s the most popular library for end-to-end testing in React Native and the one that has the most active community. On top of that, it’s the one React Native recommends in its documentation.

The Detox testing philosophy is “gray-box testing”. Gray-box testing is testing where the framework knows about the internals of the product it’s testing.In other words, it knows it’s in React Native and knows how to start up the application as a child of the Detox process and how to reload it if needed after each test. So each test result is independent of the others.

First, let’s clone a very interesting open-source React Native project for the sake of learning, then add Detox to it:

Create an account on The Movie DB website to be able to test all the application scenarios. Then add your username and password in .env file with usernamePlaceholder and passwordPlaceholder respectively:

After that, you can now run the tests:

Note that I had to fork this repo from the original one as there were a lot of breaking changes between detox-cli, detox, and the project libraries. Use the following steps as a basis for what to do:

  1. Migrate it completely to latest React Native project.
  2. Update all the libraries to fix issues faced by Detox when testing.
  3. Toggle animations and infinite timers if the environment is testing.
  4. Add the test suite package.

Setup for New Projects

Add Detox to Our Dependencies

Go to your project’s root directory and add Detox:

npm install detox --save-dev

Configure Detox

Open the package.json file and add the following right after the project name config. Be sure to replace movieSwiper in the iOS config with the name of your app. Here we’re telling Detox where to find the binary app and the command to build it. (This is optional. We can always execute react-native run-ios instead.) Also choose which type of simulator: ios.simulator, ios.none, android.emulator, or android.attached. And choose which device to test on:

Here’s a breakdown of what the config above does:

  • Execute react-native run-ios to create the binary app.
  • Search for the binary app at the root of the project: find . -name "*.app".
  • Put the result in the build directory.

Before firing up the test suite, make sure the device name you specified is available (for example, iPhone 7). You can do that from the terminal by executing the following:

xcrun simctl list

Now that weve added Detox to our project and told it which simulator to start the application with, we need a test runner to manage the assertions and the reporting—whether it’s on the terminal or otherwise.

Detox supports both Jest and Mocha. We’ll go with Jest, as it has bigger community and bigger feature set. In addition to that, it supports parallel test execution, which could be handy to speed up the end-to-end tests as they grow in number.

Execute the following to install Jest:

Six essential skills for Engineers

This is a summary of an article from research done on the six most basic skills an Engineer needs to have today.

You should probably consider this if you want to stand out and be the stand-out young Engineer you want to be.

Taking Engineering seems to be a norm that many young people consider in order to keep up with society’s demands of rapid changes, innovations and advancements in technology. Many specialised Engineering courses are also introduced and the population of people taking career paths in Engineering fields gradually grows.

Competition is getting tougher, especially for new grad Engineers. Several countries produce thousands of Engineers most of whom end up in non-Engineering fields or obliged to settle in entry levels they don’t deserve. Even if you’re an Engineering graduate, you may not be competent enough.

But don’t lose hope just yet.

Here is a checklist for you to stand out and become the stand-out Engineer you want to be.

1. AUTOCAD SKILLS

We are in the age where Engineering design is not confined to 2D blueprints or sketches alone. Designs of components, structures and architectural plans now heavily depend on 3D CAD designs especially for intricate objects in manufacturing. The ability to design with Auto CAD is really a must for Engineering and architectural people in these fields.

Having proficient CAD skills certainly gives you an advantage over other candidates. If your institution does not offer learning to use CAD software, there are crash courses offered in other technical schools for a certain number of sessions. Also, there are CAD software programs and tutorials online so you can study it yourself too.

2. BASIC PROGRAMMING / IT SKILLS

At this point, hard sciences are now converging with the digital world to be more efficient and productive. There are now industries that depend on heavy database management and automated components in industrial plants (computer-integrated manufacturing). Learning how to use programming languages and algorithms is also a plus.

Even if you’re a computer Engineering major, not all programming languages are taught in school. There are also crash courses for this such as learning SQL, Mathlab, COBOL, C#, .NET, Python, Java or Ruby. In addition, online tutorials and downloadable software programs are available if you plan to self-study.

3. COMMUNICATION SKILLS

As an Engineer, our science and mathematical knowledge or technical skills are a given. But presenting and relaying it effectively is another aspect. Being a persuasive presenter gives us opportunities to be an exceptional Engineer. We must learn how to strike the right balance between technical and communication skills as they are the keys to a successful professional career.  There are programmes and workshops that can help us enhance speech communication and presentation skills.

4. PROJECT MANAGEMENT

In our profession, we always coordinate with others or work as a team in a department. Engineers who become project managers and learn how to manage a project and a team can have a successful career. Having project management skills enables you to lead project or programme teams and will eventually allow you to master the dynamics of a project.

From there, you can handle more complex projects and be appointed in leadership positions. You can also draw other professionals together and open opportunities for business and personal growth. There are institutions offering Project Management as an elective and also a Project Management Professional (PMP) certification.

5. PROFESSIONAL LICENCE

As they say, licensure is the mark of a professional. It means you are at the top of your game and your profession. It is a standard recognised by the government, the clients and companies assuring skills and quality. It has now become a requirement in a number of Engineering firms and corporations. Having a professional licence assures the public that you can perform Engineering work. It is also seen as a commitment to your profession and gives more opportunities in your career.

6. GRADUATE DEGREES (MSc / MA / MBA / Engg D / PhD)

Today, a number of professional Engineering societies are considering making a graduate degree the mandatory credential for would-be Engineers. They require master’s degree in finance, business administration or industrial Engineering in order to advance in managerial positions. Millennial Engineers opt to take master’s degree units and certifications while working in order to raise their credentials and value of being a professional Engineer.

Some high positions need a certain depth of knowledge and expertise in a particular Engineering field so that’s where these graduate courses come into their own. Engineers must not be limited to their majors but must continue to learn and diversify into different fields.


          

When demand is down

When the economy recedes, there is a temptation to cut back on any activities that appear superfluous although in any business there should really not be any, unless they are intentionally provided to buffer the business constraint. I guess it becomes evident that when business is bad, the constraint is deemed to be in the market. That being the case, it makes strategic sense to subordinate other activities to that constraint by elevating the resource loading in marketing and sales.

Such subordination could include training up other line persons in the organization to find a supporting role in sales. Sales training should be an existing function in any organisation so the sales team could be extended to include these resources.

What about marketing, which is strictly the identification of opportunities, where sales is the exploitation of them? It is often said that everyone should be a sales person for one’s employer. Involving the rest of the team in the marketing and sales strategy can generate a lot of unexpected new options and get the spirit and drive going across the organisation.

A recent global survey, which included SA, revealed that in these slack economic times, the number of employees actively searching for a new job increases noticeably. In the case of sample SA, the results showed that more than 4 in 10 employees were seeking new positions, and our statistics indicate that this is higher than the global average by some 16%. The quarterly shift is particularly evident.

The expectation (following the big strikes and the commodity price collapse) for retrenchments in the industries affected by mining investments has been borne out dramatically. The impact on the value chain from capital investment in new mines all the way through EPCM, EPC, Consulting Engineers, main contractors, suppliers to the small business providing nuts and bolts, is a harsh reality. It becomes even more stark when we realise that SA was effectively built and developed on mining for the last 100 years.  Thus the need for radical strategy shift is urgent. A big, new marketing and sales challenge.

Essential to this is the need to define, locate and employ the human capital to operate the business. All business is doing it, but not well. My own research with many industries over some years on identifying the constraint in the business shows that there is an overwhelming belief that it exists in the lack of having the right people in the right place at the right time. This is almost a no-brainer. In my own, and in the opinion of many in the industry, is that the current process of recruitment is flawed and has been usurped by the role of HR. HR should focus and interact on the internal resources and keep them happy or at least find out what makes them tick. That might then reduce the frequency of the look-elsewhere-and-lose-staff syndrome. Recruitment is a different process requiring different skills, tools, databases, market knowledge and expertise, especially in engineering.

Until management addresses the human resource constraint with processes that protect and elevate the constraint, things will not change. It should be seen as a strategic issue at board level. It will simply be more of the same. Delays, lack of knowledge within the recruitment process, poor selection in terms of environmental/cultural fit into the organization will continue. In the days when the line spoke directly to the recruiter that understood their business, appointments were faster, better and more permanent. At least in engineering. I know, I have been on both sides. However, too much emphasis is placed on finding the perfect fit at below market value, and too little on the need for induction and mentoring in the new role. The metrics of selection are outdated and mostly inaccurate. A good start is so often messed up with poor internal conflict and dubious management skills. Hence the high turnover rate.

We are finding that matching by functional auditing is breaking the mold and needs to be seriously considered by management.


          

The currency of electronic databases for physical addresses

In the days before emails and cell phones it was crucial that one kept one’s physical address and post office address current or one was simply untraceable or able to receive written mail. Landline telephones of course were the obvious and only other mechanism and fax message depended on that facility. It was a norm that if one moved, there was the required attention paid to the physical address, and the telephone book was then invaluable.

It is evident that since the advent of emails, cell phones and now smart phones with multiple messaging options, that keeping physical addresses up to date is really only kept live by the banking systems insisting on having the FICA data but that is normally only updated when the banks call for that.

As long as print is mailed then physical addresses need to be kept current This is an issue that faces the likes of SAIMechE with the magazine, and it suffers from two issues that arise with physical addresses: change that is not notified and the inaccuracy or limited information provided by the Member which results in the item being either returned to sender of dumped at the posted office. Very often outdated or unattended mail boxes are too full to take any more mail. Problems with the efficiency and reliability of the post office itself frequently add to the amount of undelivered mail.

Address databases are renowned to be inaccurate or badly configured. I am at least three separate customers in my bank. Implementing a FICA update for the current account information does not get fed to the other accounts. It would seem that modern configuration management is a foreign practice to most banks as the format for collecting the same data varies across departments, even though the ID number is common.

It is unlikely that physical address accuracy will never be required even with all our advanced electronic communication as some form of hard mail will need to be used. It just seems that the processes for updating the address on a systematic basis is flawed. Updating of mail addresses is invariably left late so that mail continues to go to the previous address. Will GPS via mobile smart phone location ever be able to be a source of this at an accuracy that is dependable? Now that’s an app opportunity.

Let’s look at a specific case. We have about 4000 addresses to which our magazine is sent each month. On average about 5 percent are returned for reasons that the address is incorrect or cannot be located due to paucity of detail.

The best way we can keep up to date with addresses of Members is to request a regular email feedback on the receipt of the Torque newsletter on a prescribed format with the option to tick “no change” to keep it simple.


          

SAIMechE’s suggested contribution to solutions for the scarce skills challenge

At the recent workshop held by the Department of Higher Education (DHET), the President’s Infrastructure Coordination Committee (PICC) and the Council for the Built Environment (CBE), the objective was to assess the status of the scarce skills in South Africa by assembling Occupational Teams (OTs) that would be assembled largely from the voluntary engineering organizations and academia. Two days were spent following the earlier work done by the PICC/DHET/CBE team to prepare a list of what was determined by the 18 Strategic Infrastructure Projects (SIPs) to be the scare skills for this programme. This list covered the management, professional and trades identified as such by the Organising Framework for Occupations (OFO) model.

The 18 SIPs are identified at a very high level, for example SIP1 being "Unlocking the Northern mineral belt with the Waterberg as the catalyst”. For what was alleged to be "security” reasons, not much else is published on this SIP to expand on the detail, but it can be reliably gathered that a large component will be for coal mining development to take up the supply that is dwindling in the Witbank/Middelburg area.

The projected list of scarce skills, when reviewed on the basis that it should enable some effective action to commence, is in my view a rather useless piece of data insofar as the Mechanical Engineers are concerned. It simply shows "about 500” needed at a scarcity level of "20-50%” whatever that may convey.

The SAIMechE team filled in their answers to the OT list on the wide spectrum of questions and these will then presumably be assembled and evaluated for action with all the others. Most of the questions we have been addressing for years that seem to do no more than lead to the next conference or workshop. One simply comes away with the feeling of perpetual talk and no perceived action.

The first concern the team had was that the workshop called for voluntary conveners to fill variety of time consuming roles, and this just seemed to illustrate a poor business model. Here we have the National Development Programme (NDP) with its first 18 projects worth hundreds of billions of Rand in total installed value (TIV) that has no funds to pay for a properly structured resource development team that would comprise a fraction of a percent of the TIV cost. It simply illustrates the importance given to this role in the success of the programme. There is no contracted leverage with voluntary teams.

The SAIMechE team made three constructive, actionable suggestions. Firstly, establish a top level professional resources team who would be paid to get the scarce skills issue measured and solutions developed, and which would be in a position to advise the SIPs owner’s teams on the appropriate resources required at owner’s level that could be seconded from the profession in a similar way that the accounting profession does for state bodies.

Secondly, develop a model for evaluating availability of scarce skills to identify the scarce resource with the relevant, engineering specific attributes. It is based on the Engineer’s credo that you do not know much about anything until you can define and measure it. Scarcity needs to have two references to be measureable: what you have and what you need. Accordingly SAIMechE could offer to facilitate working with professional engineering resource recruiting bodies to create a large and well configured, best-in-class, dynamic database of engineering resources for these and other South African projects. By simply stating that we need a number of Mechanical Engineers does not resolve the issue. The only time the real scarcity is known is when the employer specifies the need at granular level of definition. For example one can search on Mechanical Engineers and get hundreds, but then ask for those with the specifics such as 10 years of coal plant processing experience or conveyor and coal chute design, one may be lucky to find a few if any under today’s dwindling expertise that largely exists in near retirement age groups or who may have emigrated.

Thirdly, with this information and working with the employers undertaking the projects and who issue the job specifications with these details, we can identify those who should be taking on the Candidate Engineers that wish to do their training under the SAIMechE Professional Development Programme. SAIMechE would be the paid conduit to provide information on scarce skills to the PICC or the CBE on an on-going basis enabled by the dynamics of the model. We need to ensure that the expectation of perpetual voluntary work by professionals is not presumed. It’s a business reality to pay for value. It would it addition be of value to be able to second experts from the Membership, exploiting the immense, collective intellectual capital of the Institution and effectively meeting the essence of the mission statement.



          

That was the year that was

An interesting year, that puts it mildly. How else would one describe a year where the global economy has the sign of regressing into a second recessionary phase, the USA and Europe playing dangerously close to debt collapse, the Arabian spring syndrome spreading globally, the biggest earthquake created Tsunami in our history with its subsequent nuclear event, an observable widening of the wealth gap both in SA and internationally, and our own political scenario showing quite clearly government capacity to do anything constructive is deteriorating further.

Locally, from the perspective of engineering resources, a somewhat dichotomous situation exists. The scenario would be quite impossible to have envisaged let alone planned. Following what appears to be a world-wide trend, infrastructure in SA has been neglected, used and not maintained to the point where much of it must be in need of full replacement. A lot of the manufacturing industry is talking of reaching a point where SA appears to be de-industrialising, Eskom’s six packs under construction get later each month, and there is a "shortage” of skilled and experienced engineering resources. The use of the word shortage needs to be qualified as there are many retired or retrenched resources that are fully able to deliver but are overlooked for social or customary reasons of age. Adequate training up of new resources is dismal. The well used metric of Engineers per million head of population must make some sense as we are in the lowest league here and I guess it is both confirmed and its impact evident in the abysmal lack of technical resources in government, provincial and municipal structures. If one is envisaging a turnaround in infrastructure status, then where are the resources to achieve this going to come from? Who is going to be acting in the role of the Owner’s Team for major infrastructure projects?


PPS recently published a finding that some 600 professionals were leaving SA a month, many of these engineering resources essential to build our own environment. Included in these are mining and project management skills that are leaving our commodities industry to boost those in Australia, Canada and South America in particular. With an estimated R2,5 trillion of mining wealth buried in our SA earth awaiting recovery and even beneficiation, this loss is almost self inflicted damage. If government does not sort out the noise on the nationalization issue, more losses will occur through the realization that it will seriously deplete the mining industry and expand unemployment.

We are producing many graduate Engineers, Technologists and Technicians that are unable to usefully progress into positions where they are trained and mentored to become competent and productive resources. Unless the recent strategic advisory and action committees being developed by ECSA together with the Voluntary Engineering Associations are able to progress rapidly to a point of active execution, the situation will simply deteriorate further. This requires the essential and proper use of the skills development levies paid to the national skills fund that have accumulated and not been used through sheer bureaucratic lack of vision and mismanagement. Herein lies the opportunity for many retired or semi-retired engineering resources to participate on a paid basis as mentors in properly managed, competency-based outcomes training and development programmes.

Engineering resources remuneration levels are rising. Supply and demand is driving this as particular positions become seriously underprovided with candidate options. How should a business that requires the right people in the right place at the right time go about recruiting resources? When supply is erratic or constrained, then focus on the constraint and subordinate other activities to it. Find the buffers to protect the constraint from starvation or blockage. A supply buffer will be the provider who has access to the best database and network of resources and who can filter out the signal from the noise of the conventional practice of multiple inappropriate CVs from multiple providers all drawing from the same pool. Change HR’s customary structures. Engineering business should look hard at their people procurement practices.


          

Stories from the engine room (Feb 2011)

I have decided to relate some experiences from my current business activities that consist of a fair mix of recruitment and placement of engineering resources, training, mentoring and consulting assignments, and some services to the Institution and the Engineering Council. All in all, it mixes pretty well, with the common focus being on engineering skills acquisition, evaluation, communication, collaboration, development and the steady building of a large, live database of new graduates of all disciplines and active and retired engineering resources . The major part is interfacing. with employers and candidates. Working in the engine room so to speak on a daily basis provides one with up -to-date information, change patterns, trends, behaviours and the supply and demand dynamics directly affecting the profession. So hopefully this is of some value. My very limited story, in a note form with comments, seems the most appropriate way to set this out.

  1. There are far too many agencies in the recruitment space. Many are not more than body shops, collecting CVs and submitting them out to all and sundry with little care as to job and profile alignment. Most claim to have expertise across many varied professions and trades. Few do as we do, focus only on the engineering profession and provide guidelines and assistance to employers and candidates as Professional Engineers can and should do. We supply the people part of the engineering business, just as other experts provide equipment.
  2. HR does enormous harm to the process of procuring Engineers. Employers who normally take great care in specifying a product, seeking out reputable suppliers, visiting their works and assisting the technical buyers will somehow avoid involvement and leave HR to do the work of handling dubiously worded job specifications, at times we have counted, to up to 40 separate agencies for one post! Why not align with selected specialist consultants who know the product and the profession, particularly in pre-planning future needs?
  3. A 24 year old recruiter in an agency will interview a 53 year old Professional Engineer and advise him he is too old for the job. It happens . If that is not the height of insult then what is? Our greatest engineering talent exists at this age and beyond, and it is being neglected at great risk of losing experiential skills.
  4. A common feature is how many candidates simply cannot spell, edit a CV, read the job specifications and who will apply for a position shortly after graduating that specifies 10-15 years experience in an engineering management role. This "give it a go” attitude can often comprise the bulk of applications.
  5. The time is past that employers, perhaps through the naivety of HR, can expect Mr or Ms Perfect to be standing on the street corner waiting for their call. The good ,experienced skills are in short supply and fully employed, are internationally mobile and proving it, and demand high remuneration to the surprise of the prospective employers who claim the candidate be to out of touch with the market. Well, I am not sure where HR gets their "market levels”, but invariably from historic tables issued a while ago with somewhat wide and unhelpful margins and weak descriptors. The difference is so evident that we are considering publishing a real-time remuneration survey for interested employers based on our own up to the minute information for the engineering profession, a sort of real-time remuneration survey.
  6. Then we must take up some concerns with line management. How many are really equipped to make a valid judgment from a CV alone? Many are so busy that they have not viewed them or done so in such a hurry with no reverting questions and simply accept or reject them. I have witnessed a small pump getting more adjudication time than a senior engineering candidate!
  7. Near-fit of candidate to the specified role does not succeed. The belief that Mr or Ms Perfect must be found is the only way. We have no time to train up the near-fits. I am appalled at the lack of training, mentoring and acceptance of this within the crisis we face in scarce skills today. Industry needs a major wake-up if it is to collectively be in a position to manage a serious growth development in the economy.
  8. If our own SA Engineers knew the remuneration that foreign expats are earning on our Eskom projects, they would have a fit. And many of our own are equally able to provide the skills and expertise.
  9. As Engineer Placements, we are working closely with Voluntary Associations and ECSA to develop structured training and mentoring for the EIT programme required after graduation. The intent is to seek out retired Engineers to act as paid mentors with programmes endorsed via the SAIMechE Professional Development Programme with funding from the revised NSDS3 and industry. At least Minister Ebrahim Patel, in his call to train 30,000 more Engineers, should be grateful and hopefully actively supportive. Pigs may fly though.

          

The Devil Is in the Details

In response to “stranger danger” fears, India has become the ninth country to adopt a sex offenders’ database, with personal details of those convicted for rape and sexual assault. The year-old database contains 500,000 names and will be used for “regular monitoring and tracking” by the police, the government says.

Strong efforts at every government level are critical to prevent sexual abuse and prosecute culprits-and officials cited a serial offender to justify this database. But latest crime figures show that in almost 95 per cent of the 38,947 rapes recorded in 2016, the accused was known to the victim. In the absence of better law enforcement and adequate safeguards, the database will do little to advance change.

The government is under pressure from public demands to better address rampant sexual violence. But instead of populist or short-term technological solutions, it needs to do the hard work of breaking entrenched structural barriers that survivors face in reporting these crimes and obtaining justice.

In my 2017 report for Human Rights Watch, “‘Everyone Blames Me’: Barriers to Justice and Support Services for Sexual Assault Survivors in India,” I found that sexual assault survivors, particularly from marginalised communities, still face formidable obstacles when reporting to police. They often suffer humiliation at police stations and hospitals, are subjected to degrading medical exams, such as the “two-finger test,” and have little protection from retaliation by the accused. They face significant barriers to obtain healthcare, counselling and legal aid. The government has yet to put in place effective mechanisms to prevent child sexual abuse. The challenges are even greater for women and girls with disabilities.

Studies by Human Rights Watch and the American Civil Liberties Union show public sex offender registries in the US have done more harm than good. They frequently lead to harassment, ostracism and violence against former offenders and impede their rehabilitation.

The Indian database for each entry includes the name, address, photograph, fingerprints and details of Aadhaar card, which carries biometric data. For now, the government says, only law enforcement officials will have access. However, officials confirm that in future, the database will include people who were charged, but never convicted. Making this database public has not been ruled out.

The government’s invite of bids from companies to develop this database said it would include details of all those arrested, charged or convicted of sexual offences, including children. Information on the convicted offenders would be public while law enforcement agencies would have access to the rest. It proposed categorising the individuals as “low,” “moderate,” or “high risk.” Whether or not a person gets added can depend on archaic laws open to misuse. The government’s recent move to acquire an automated facial recognition system to identify criminals-and ostensibly improve policing-exacerbates concerns regarding privacy, surveillance and civil liberties.

Rape is under-reported in India, largely because of social stigma, victim-blaming, poor response by the criminal justice system, and the lack of any national victim and witness protection law. Rape survivors are highly vulnerable to pressure from the accused and police not to report. Child victims are even more vulnerable to family and societal pressure.

The government says the database will not compromise privacy, but the absence of laws to protect privacy and data in India raise further concerns. With vigilante violence on the rise, a data breach or rumours of inclusion in the registry is especially dangerous. Little is known about the dissemination of database information and whether it could perpetuate discrimination. Would a private employer seeking a background check on a prospective employee get this information?

To address sexual abuse, the government should work closely with women’s and children’s rights groups to reform the criminal justice system and promote a survivor-centred approach, focusing on evidence-based solutions. A sex offender registry is not the answer.

Zato Blog: Configuring Zato for high-performance Oracle Database connections

If you need to configure Zato for Oracle DB connections and you want to ensure the highest performance possible, this is the post which goes through the process step-by-step. Read on for details.

Overview

Note that Zato 3.1+ is required. The tasks involved are:

  • Installing Zato
  • Installing an Oracle client
  • Greenifying the Oracle client
  • Starting servers
  • Creating database connection definitions
  • Confirming the installation

Installing Zato

  • Choose your preferred operating system and follow the general installation instructions - Oracle DB connections will work the same no matter the system Zato runs on

  • Create a Zato environment with as many servers as required. Again, there are no Oracle-specific steps at this point yet.

Installing an Oracle client

  • Download an Oracle client and install it on all the systems with Zato servers

  • Add the client's installation path to LD_LIBRARY_PATH. For instance, if the client is installed to /opt/zato/instantclient_19_3, add the following to ~/.bashrc:

    export LD_LIBRARY_PATH=$LD_LIBRARY_PATH:/opt/zato/instantclient_19_3

Greenifying the Oracle client

  • This step is crucial for achieving the highest performance of SQL queries

  • For each Zato server installed, open its server.conf file and find stanza [greenify]

  • If there is no such stanza in server.conf, create it yourself

  • Modify the stanza to greenify the libclntsh.so library - this is a library that the Oracle client ships with. For instance, if the client's installation path is /opt/zato/instantclient_19_3, the full path to the library is /opt/zato/instantclient_19_3/libclntsh.so

In addition to my bug fixes and performance improvements, this release improves in many place the user experience. It also extends a lot the existing workflow to support more use cases. We see 8 new modules landing as official.

You can have a try on the demo server, use the docker image or download it here.
As usual the migration from previous series is fully supported. Some manual operation may be required, see Migration from 5.2 to 5.4.

Changes For The User

We can show a visual context on the rows or cells. Those visual context can be muted , success , warning or danger. Many modules have been updated to use them like the payable and receivable today on the party or when an invoice is due etc.

In the search bar of the clients, we enabled the direct search on fields of relational field types, like One2Many, Many2Many and Many2One. This is done by appending a dot to the relational field name and then the name of the field in the relation model. E.g. On products filter you can use the search clause Variants.Code: PROD, to find all products, which variants have a code named PROD.
The search entry provides completion for such related fields.
By default only one level of completion is activated but customization can activate more. This feature also works on the keys of dictionary fields like the product attributes.

Now the clients display a more user-friendly error messages for domain validation error. The clients display the exact failing constraint using the same format of the search bar.

All the list views have been reviewed. The most important fields are now expanded to take advantage of available space. For main editable view, the creation of new record is done on top instead of bottom to avoid to load all the records.

For now, an CSV export created by a user will be only available for him by default. The administrator can make an export available to a group of users.

Desktop Client

Now, when drag&drop is available on a view, we show a draggable icon to notify the user but also to provide a handle to drag which is easier when the list is editable.

Web Client

The web client now supports drag and drop to order list and tree rows like in the desktop client. There is one small difference to insert a row inside a non-expanded row: The user must drop it below the row while pressing the CTRL key. Otherwise the row is dropped next to the row.

The column size of the web client has been improved. Now columns have a minimal width (depending of the type) and a double scrollbar (top and bottom) is displayed if there is not enough space to show all the columns on the view-port.

Accounting

The constraint that prevented to use twice the same invoice sequence per fiscal year, has been relaxed. Now Tryton checks only that the sequence was not used to number an invoice with a later date.

In addition to the country, the tax rules can now be written using the subdivisions of origin and/or destination. A child subdivision will match the rule based on an upper level subdivision. This is useful for countries that have different tax rates for some subdivisions.

The income statement is included in the balance sheet for the Spanish accounting (as it is done for other countries). So the running income of the current year is already included before the year closing.

Bank

The BIC of banks are now validated and formatted. This avoid encoding error and ease the research.

Party

Until now, the subdivision on an address was limited to the top-level subdivision of the country. It is now possible to define which types of subdivision are allowed to be used. Tryton comes with some configurations following the country rules for postal address format.

Product

Now it is possible to configure a sequence for the product code that will be used to be filled at creation time. This may be used to ensure to have a unique code per product, even when it is duplicated.

Like for parties, we added on product a list of identifiers. By default, Tryton supports and validates these numbers: EAN, ISAN, ISBN, ISIL, ISIN and ISMN. Non-standard identifiers are supported also. These identifiers are used for matching when searching products by name.

The product cost price can be used in the price list. It uses the cost price of the company set in the context. This allows to build price lists by defining a margin to apply on the cost.

You can now define which unit of measure is the basis for quantities used in a price list. In standard modules we support the default unit (the original one) and the sale unit.

Purchase

It is now possible to configure the customer code of the current company on the supplier party. The code will be displayed on the request for quotation .

The same processing delay for purchases is added to the requisitions. This allows to reset an approved requisition to draft if it was not yet automatically processed.

Sale

We added the same processing delay for sales to the sale complaints. So you can reset to draft a complaint after being approved or rejected if it was not yet automatically processed.

We added an option to deactivate a subscription service. This prevents to use these service for new subscriptions.

We allow now to finish a subscription line before the next consumption. This gives more flexibility about ending subscription.

Stock

Now users are able to set a default warehouse in the preferences.
This is useful for companies with multiple warehouses. It saves time for the users as they could have the warehouse filled-in for which they work.

You can use consumable products in an inventory if needed. There are still no requirements and the inventory is not automatically filled with products of this type.

When you are looking at the evolution of the stock quantity for a product, you can open the date to see the moves involved for the changes.

When opening the graph of product quantities by warehouse, if there was no move for the current date, the user can not see the current quantity. Now we add always an entry for the current date.

We force now to always have a minimal quantity for the order point. This avoid confusion for the case where it was not set. Now if you do not want to trigger the purchase or production for any quantity, the user must set an explicit negative quantity.

Timesheet

When using the “Enter Timesheet” wizard, now we display the date in the window name (next to the employee name). The shown date is the one selected in the first step of the wizard.

New Modules

Secondary Unit

These modules allow to define a different secondary unit and factor on the product for sale and for purchase.
The quantity of sale and purchase lines can be defined using the secondary unit fields (quantity and unit price), the main unit fields are automatically updated using the product factor.
On related documents like the invoice or shipment, the secondary fields are displayed using the factor stored on the sale or purchase.

Amendment

The amendment modules allow you to change sales and purchases that are being processed and keeping track of those changes. When an amendment is validated the document is updated and given a new revision. If needed, the invoices or shipments are also updated or recreated to match the new order.

Purchase and Sale History

These modules activate the history on sales and purchases but also add a revision number which is incremented each time the document is reset to draft. The revision number is appended to the document number to ensure parties are communicating about the same version.

Changes For The Developer

It is now possible to use SQL expressions as value with the create/write methods. The main purpose is to be able to use the time functions of the database server which are linked to the transaction instead of the one provided by the Tryton server.

The expand attribute has been changed from a Boolean (1 or 0) into an integer. The integer represents the proportion of available space which is taken among all expanded columns.

The PYSON Eval now supports the dotted notation. This feature is a common expectation from beginners. So we decided it is good to support it.

We have already a multiselection widget to use with a Many2Many field. But now we have also a MultiSelection field which stores a list of value as a JSON list in the database. This is useful when the selection has a few options. For now, the widget is also available on list views (but not editable). And the field is usable in the search bar of the client.

You can now define a different start date when using PYSON Date or DateTime with delta.

Now we give the possibility to define a different order (alphabetic) to the keys of a Dict field.

Even if cron jobs are relaunch, it is better to retry directly them few times when a DatabaseOperationalError is raised. This also avoid unnecessary errors in the logs.

Accounting

The generic checkout page for Stripe has been updated to use Stripe.js and support setup and payment intent.
Until now, it was required to setup a webhook from Stripe to Tryton in order to receive the events to update asynchronously the workflow of the payments. Now if you do not setup such webhook, a cron task will fetch periodically the new events and process them. This is useful for testing or when Tryton can not be reached from outside.

We require now to have a fresh session to post a statement. If it is not the case, the client will request to re-enter the user password (or any other authentication method configured).

Country/Currency

The countries, subdivisions and currencies are no more loaded from XML at the module installation but using proteus scripts which use pycountry data: trytond_import_countries and trytond_import_currencies . The translations are also loaded by those scripts.
This reduces the maintenance load of each release and allows users to keep their database up to date without relying on Tryton releases.

Party

As the countries are no more managed as XML data in Tryton (but by import script). The address format are now using country and language code instead of Many2One. So a format can be created even if the country or the language do not yet exist.

Stock

As we now keep a link between the inventory moves and the outgoing moves, we simplified the synchronization algorithm to use this link. Another advantage is that if the product is changed on the inventory move, the outgoing move is also updated instead of creating a new move.

Duties and Responsibilities (essential functions): A) Develop and produce daily data using current in-house and third-party software applications, including (but not limited too) Excel, Access, BIRST, RMA, etc. B) Independently coordinates the daily engineering analysis and reporting for KPI metrics. Qualification Requirements: A) Knowledge and Skills: Must have basic knowledge of database programming language (Excel, Access, etc.) Must have basic knowledge of data collection and analysis practices. Must have basic knowledge of statistical measuring and analysis practices. Ability to work across functional boundaries, with excellent analytical, problem solving, communication, and presentation skills. Able to communicate clearly and build strong relationships across engineering groups. President Trump nominates Stephen Hahn as new FDA commissioner

President Donald Trump is expected to nominate Dr. Stephen Hahn to be the next head of the US Food and Drug Administration.

The acting commissioner of the FDA, Dr. Ned Sharpless, will return to his role as the director of the National Cancer Institute, according to a press release from the US Department of Health and Human Services.

By law, Sharpless was only allowed to serve a total of 210 days under the Federal Vacancies Reform Act. His tenure ended Friday at 5 p.m. ET.

Hahn is a well-known radiation oncology expert and is the current chief medical executive of The University of Texas MD Anderson Cancer Center in Houston, where has been a professor of radiation oncology since January 2015.

Asked to describe himself when he was appointed to the leadership position at MD Anderson, Hahn used one word: "Energetic."

The 59-year-old will need that energy to run an agency that employs more than 17,000 people and has a $5.7 billion budget.

The agency protects public health by regulating much more than the food and drugs mentioned in the department's name. The FDA regulates medical devices, tobacco, alcohol, veterinary products, vaccines, blood, microwave ovens, nail polish and much more.

Helping run the daily operations of MD Anderson, Hahn currently manages a $5.2 billion operating budget, 20,300 employees, 7,000 trainees and more than 3,000 volunteers, according to the hospital.

MD Anderson is one of the world's largest cancer centers. It is ranked No. 1 in the United States for cancer care, according to the U.S. News & World Report "Best Hospitals" survey.

It cares for more than 141,000 patients in 2018. It is known as a leading cancer research center, earning the most number of grants from the National Cancer Institute, according to the center. Its staff even includes a 2018 Nobel Prize winner.

At MD Anderson, Hahn had a good reputation for turning its budget around. The center faced serious financial losses in 2016, according to the Houston Chronicle. The paper said when Hahn noticed the problem, he "laid out a slide presentation to top officials that showed the elite Houston hospital was on track to lose up to $450 million in the new fiscal year without swift and decisive action."

"We were in a financial crisis, if you will, and that we had to move in a certain direction," Hahn told the Cancer Letter in 2017 in an extensive interview about that time in his work. He oversaw the largest layoffs in the cancer center history, but the center moved back into solid financial territory.

Money challenges weren't the only test of Hahn's leadership. This year, when MD Anderson received notification from the Centers for Medicare and Medicaid Services that it could become ineligible for reimbursements after investigations found violations with its labs, Hahn publicly promised to fix the problems and took responsibility. MD Anderson explained that corrective action is ongoing, according to a letter MD Anderson sent CMS.

Hahn was first brought to MD Anderson in 2015 to run its radiation oncology division and was appointed chief medical executive in June 2018.

Before his work in Texas, Hahn was chairman of radiation oncology at the University of Pennsylvania School of Medicine from 2005 to 2014. As vice chairman for research, he led the expansion of the department's research base, which regularly ranked first or second for NIH funding, according to the school.

In 2009, Hahn testified on Capitol Hill, taking responsibility for a doctor who worked for him who had botched the prostate cancer care of dozens of military veterans, according to Hahn's testimony. He asked the doctor to resign and created a new system to prevent future mistakes.

As a clinician, Hahn specialized in treating lung and genitourinary cancers. He is listed as an author on more than 200 peer-reviewed journal articles and other publications that focus on topics including proton therapy, radiotherapy and cost-effective treatments.

In the early 1990s, Hahn worked in Santa Rosa, California, as a medical and radiation oncologist, and took a leadership role early in his career as chief resident of internal medicine at the University of California, San Francisco. He worked from 1993-1995 in Bethesda, Maryland, as chief of the National Cancer Institute's prostate cancer clinic.

Hahn graduated with a bachelor's degree in biology from Rice University in 1980. He went to medical school in Philadelphia at Temple University, graduating in 1984.

The same year he graduated, Hahn married Lota Leigh Morning at the First Presbyterian Church of Allentown, according to the local paper at the time. Hahn told MD Anderson that the worst advice he was ever given was to "never get married."

"If I'd listened to that advice, I'd have missed spending my life with my best friend," he told MD Anderson.

The couple have four children and a rescue dog.

Their online presence shows a love of wine and travel to Italy. He told MD Anderson that he gets up at 4 a.m. every day to read and work out, and if he wasn't a doctor, he'd be running a vineyard.

Hahn has been a Republican donor, according to Federal Election Commission records. Unlike some other FDA leaders, Hahn has received very little money from the medical device and drug industries. According to CMS' Open Payments database, Hahn received $1,021.40 in lodging and travel money from a radiation oncology device company in 2017 — much less than others in his field, according to the report.

To get the top FDA job, Hahn will have to be approved by the Senate. He would inherit an agency that since March has been going through an agency-wide reorganization.

Sharpless has been the acting FDA commissioner since April and had the backing to become the agency's permanent head by dozens of medical associations and past FDA commissioners who sent letters to the White House advocating on his behalf in September.

The agency is managing a vaping crisis that has killed at least 37 people nationwide and has raised real concerns about the safety and regulation of e-cigarettes. The country's opioid crisis has 1.7 million Americans struggling with addiction and there were more than 70,200 overdose deaths in 2017, according to the CDC. There are also challenges involving delays in drug approvals, as well as problems with the manufacturing safety of drugs.

On Friday, medical associations praised Hahn for his work.

"For several years, I served with Dr. Hahn on a National Cancer Institute study section, and I always found him to be an extremely thoughtful and science-oriented grant reviewer," said American Association for Cancer Research President-elect Dr. Antoni Ribas, in a statement. Ribas is a professor of medicine at the University of California, Los Angeles. "He has always put the interests of patients first, by embracing the importance of regulatory science to truly inform and improve the way new cancer medicines are evaluated for safety and efficacy."

"We welcome the nomination of a permanent FDA commissioner at a time when the agency's role to protect and promote public health is only growing in importance," said American Heart Association CEO Nancy Brown in a statement. "Should Dr. Hahn be confirmed, we look forward to working with him to aggressively advance public health priorities in areas including tobacco control, nutrition, drug and device development and review and more."

Not all people know that it is possible to run queries on other Azure SQL Databases. Normally with on-premise databases, we are used to use the following cross database query :

But it will run into an error :

From Microsoft : "The solution is using elastic queries. The elastic query feature enables you to run a Transact-SQL query that spans multiple databases in Azure SQL Database. It allows you to perform cross-database queries to access remote tables, and to connect Microsoft and third-party tools (Excel, Power BI, Tableau, etc.) to query across data tiers with multiple databases. Using this feature, you can scale out queries to large data tiers in SQL Database and visualize the results in business intelligence (BI) reports"

  ㅇ 비관세장벽 세계적으로 증가 추세이나 관세장벽은 감소

    - 자유무역협정(FTA)의 확대로 관세장벽은 점차 낮아지고 있는 반면 비관세장벽은 기술표준, 건강과 안전, 환경보호 등을 근거로 강화되어 FTA 활용을 저해하고 있음.

    - '2019 아시아태평양 무역과 투자' 보고서에 따르면 지난 해 세계무역기구(WTO)에 보고된 전 세계 신규 동식물위생검역조치(SPS)와 무역기술장벽(TBT)이 3466건으로 전년보다 16% 증가함.

 

  ㅇ 2018년 3월 기준 아세안 10개국 모두에서 비관세장벽이 2015년 대비 약 15% 증가

    - 아세안 국가에서의 비관세장벽은 기술적 조치(Technical Measures)가 대부분을 차지하며 동식물위생검역조치(SPS)*는 농산물과 식품에, 무역기술장벽(TBT)**은 비식료품에 집중됨.

    - 태국, 말레이시아, 브루나이, 미얀마의 경우 전체 비관세장벽 중 30% 이상이 동식물위생검역조치인데 반해 캄보디아, 인도네시아, 라오스, 싱가포르, 베트남의 경우 무역기술장벽이 더 많은 것으로 나타남.

What explains European Union international contestation of justice and values in the emerging global order?
To answer this question the research provides a systematic, unified, practice-oriented analysis of EU contestation through three overarching objectives:
(1) it analyses the EU’s promotion and contestation of global justice and a value-based global order through the use of a unified research design built on a database across four fields of bilateral, minilateral, multilateral, and transnational global engagement;
(2) it draws together systematic data from both EU and non-EU governmental, non-governmental, and transnational actors in order to ascertain the influence of facilitating and restraining conditions, as well as institutional and legal factors, in shaping the EU’s contribution to global justice and a value-based global order;
(3) it identifies and develops best practices of EU promotion of values and global justice, and through draft evidence-based policy-recommendations engaging in proactive communication and dissemination activities.
The research consists of research teams taken from the world’s top four “EU as global actor” centres, whose scientists have excellent expertise on the EU as a global actor, non-European global actors, issue-specific fields, and methods, delivering conceptual and theoretical innovation, knowledge-transfer, and evidence-based, practical recommendations.
